You may EITHER complete the form online and pay online OR download, complete, print and mail it with a check.
You may download, fill out, print and mail your completed form, along with a check/money order for $75 per person, payable to BCHS 67 to: Ricki Stolz Somers, 85 Marsdale Court, Selkirk, NY 12158. You may complete the downloaded form on your computer, tablet or phone and then print the completed form or you can print the form first and then handwrite your information on the form.
